Create, investigate, communicate, and resolve operational incidents with a durable timeline.

Automatic incidents
- A monitor-created incident identifies its creator as Monitor or FaultPilot automation—not as a human user.
- It opens only after the monitor's failure threshold is satisfied.
- Recovery can update or resolve it after the configured success threshold.

Manual incidents
- Create a manual incident when impact is known through support, deployment, or another provider.
- Choose affected monitors, severity, current lifecycle state, and a concise customer-safe issue summary.

Updates and timeline
- Add operational updates as investigation progresses.
- Keep raw response evidence private and publish only appropriate status-page communication.
- Use timestamps in UTC where teams cross regions.

Resolution
- Confirm the service is genuinely stable before resolving.
- Record the cause and corrective action when known.
- Use a post-incident review for recurring or high-impact failures.
